Day care service centre for people with dementia, New Horizon Centre, has four locations in Singapore, including one within Tampines (Blk 362 Tampines Street 34).
Operated and supported by Dementia Singapore (formerly The Alzheimer’s Disease Association), the centres offer day care services for people with dementia, as well as an Early Dementia Programme for those at an early stage of dementia.
The objectives of the centres is to provide relief for caregivers, to increase families’ abilities to cope with & continue caring for people with dementia (and/or physical disabilities), and to provide nursing care, occupational therapy, physiotherapy, and social activities.
Tampines.
At each of the centres, the staff consists of a team of medical professionals that includes doctors, occupational therapists, physiotherapists, and nurses.
Each individual is considered unique and valuable at the centres, which is why there is an individualised care plan for each person: clients are assessed to determine their physical, cognitive, and social functioning in order to put together an effective therapeutic care plan.
Core activities at the centres are Occupational Therapy and Physiotherapy, Group Exercises, Group and Individual Games, Reminiscence Therapy, Leisure Activities and Hobbies, Outings and Excursions, plus Family Involvement.
The involvement of the patient’s family is crucial for the operations, hence regular feedback is given to the family, and caregivers are also encouraged to participate in the centres’ Caregiver Support Groups, where information and sharing on caring for a person with dementia are provided.
